Medical Case Management

Medical Case Management provides coordination for medical appointments and supportive services in order to stay engaged in care, maintain treatment, and achieve personal health-related goals. Case Managers are advocates to help clients achieve their health goals, address barriers to care, and connect to resources. Services include thorough assessment of needs, client-centered service-planning, information, linkages to additional services, assistance with applications social services/benefits, coordination of care, and health literacy. Providing this support and addressing challenges can help individuals continue daily treatment and stay virally undetectable.
